          With reference to the above-mentioned subject, I wish to inform that in view of the 
          current Covid -19 situation, the Government has decided to start the teaching sessions 
          for students in school on condition of following S.O.P. (Standard Operating Procedure) 
          set by the Government.

          have thoroughly read and understood all the details given in S.O.P. regarding the role 
          of parents/ guardian.  
           
          I hereby, give consent to my ward to attend regular classes/examinations/practicals in 
          school. w.e.f. 25/01/21.  I promise that, all the rules and regulations in concern to 
          Covid-19 and S.O.P. set by the Government will be strictly followed by my ward. My 
          ward has been orientated by me to wear mask at all times, carry his/her food and 
          drinking water and do not share it with others. I also ensure that if any member of my 
          family tests positive for Covid-19 or my residence comes under containment zone, then 
          I will not send my ward  to school.
