Instructions to Raters: |
Note: Consultant Evaluations are performed annually with the rating period being January 1st through December 31st. However, for Project Specific Agreements, if the consultant has performed less than $300,000.00 of work, or has had less than 2 months of work, a rating is not required unless the rating is the final rating. For Term Agreements, an evaluation for each task is required |
ONLY ENTER INFORMATION ON EACH SHEET THAT IS SHADED IN BLUE. "ANY CELLS THAT |
CONTAIN A RED TRIANGLE PROVIDES ADDITIONAL RATING GUIDELINES IN A POP UP |
FORMAT." |
1. Fill out all portions of the General Information sheet that are shaded in blue. If a question is not applicable, enter "n/a". |
2. Enter a rating (1, 3 or 5) for Question 5 of the PM Rating using the PM Rating Guidelines. |
The Regional Consultant Coordinator (RCC) will enter ratings for Questions 1 - 4 and 6 - 8 |
3. Enter a rating (1- 5) for each question on the Quality Rating sheet using the Quality Rating Guidelines. |
If a question is not applicable, enter "X" in the n/a column. |
Question numbers 1-9 should usually apply in all circumstances discuss with the Regional Consultant Coordinator if necessary |
Questions 1, 2, 5, 6, 7,19 and 21 REQUIRE the rater to submit a detailed justification if the rating is >3. Simply citing the rating criteria is not acceptable justification. The rater must cite specific examples of performance. |

Questions 12 & 13 are applicable if staff is responsible for office engineering functions |
Questions 14 & 15 are applicable if staff performed asbuilts |
Question 16 is applicable if staff includes a Schedule Analyst or an RE |
Question 17 is applicable if staff includes a supervisory personnel (i.e. NICET IV or RE) |
Question 16 - 21 are applicable if staff includes an RE |
4. You must provide comments and explain all ratings of 1's and 5's on the CES sheet. |
If your comments require additional space, provide them on a separate sheet, and note "see attached". |
5. On the CES sheet, enter the Rating Type (final or interim) |
6. E-mail the completed file to the Field Manager for review |
7. If the Field Manager concurs with the ratings, the Field Manager will |
E-mail the file to the Regional Consultant Coordinator (RCC) |
8. The RCC will review the Quality ratings and complete the PM rating |
After completing the ratings, the RCC will enter the ratings in the CES Ratings Summary Form |
9. If the total rating for an agreement is less than 3.80, the RCC will e-mail a copy |
of the ratings to the Consultant. The Consultant will be afforded the opportunity |
to appeal the rating with the Rater and Rater's Supervisor. |
10. The RCC will print a copy of the completed CES form, and e-mail a copy to the Quality Rater |
for the Quality Rater's files. |
