| Kindly complete this form and send it to esmoasia@mci-group.com. The hotel request will be processed and the Housing Bureau will confirm the availability with you. An invoice will be submitted to you. A credit card authorisation form or a weblink will be sent to you to guarantee the booking. Upon receipt of your payment, we will send you an email with a rooming list template to enter the personal details of each of the participants of your group. You will have until Tuesday, 16 October 2018, midnight AEST to complete the submission of your list of participants. |
