               What supporting documents do I need to submit? 
               There are four (4) required supporting documents: 
                      Signed Application Form (The application form is attached to your “Application Submitted” 
                      confirmation email.) 
                      Motor Vehicle Purchase or Lease Agreement 
                      Massachusetts Certificate of Registration 
                      Massachusetts Driver’s License (If you do not have a MA driver’s license, please contact program 
                      staff for alternative proof of residency options.) 
               How can I avoid common mistakes? 
               Submitting incomplete or incorrect supporting documents is the most common reason that MOR‐EV 
               applications are rejected.  We are happy to help you resubmit corrected supporting documents, but 
               rejected applications take longer to process, delay approval, and may even result in having your 
               application cancelled. Use this checklist to help avoid the most common mistakes. 
               Application Form  
                      My application form is signed. I only need to submit the signature page; the terms & conditions 
                      are for my records. 
               Purchase/Lease Agreement 
                      My purchase/lease agreement copy is complete with no missing pages.   
               Proof of Registration 
                      All information is visible on my registration.  
               Massachusetts Driver’s License 
                      My driver’s license is not expired, and I can read all information. 
               All my supporting documents are: 
                      Complete – Portions of my document are not cut off or missing.  
                      Legible – I can read all information. 
                      Correct – I’ve viewed examples of correct supporting documents.

               Signed MOR-EV Application Form 
                      The scan or photo of your application form must be legible and complete.  
                      Your application form must be signed. 
                      The terms & conditions page is for your records. Only the signed application form needs to be

               Motor Vehicle Purchase/Lease Agreement 
                      The scan or photo of your purchase/lease agreement must be legible and complete. 
                      There are many different types of purchase/lease agreement depending on the dealership and 
                      automaker. This sample below is for reference only; your purchase/lease agreement may look 
                      different. 
                      At a minimum, a complete purchase/lease agreement will include:  
                         *  Date of purchase/lease 
                         *  Name of the purchaser/lessee 
                         *  Year, make, & model and vehicle status 
                         *  Vehicle Identification Number (VIN) 
                         *  Price of the vehicle  
                         *  Lease term, if applicable
